HOW TO BECOME A NEW PATIENT

We value transparency and want our patients to know what to expect when they join our practice.

Next, complete these 2 forms, Consents and Health History Form. Please be thorough and include dates when applicable.

Christy our office Manager will reach out to you to schedule your first visit within 1-2 business days. New Patient Appointments are 1 hour, available Monday through Friday.

• Bring recent lab results (within the past year) to help establish a baseline if available.

• Bring recent Mammogram, DXA scans, or other relevant testing/imaging results (within the past 3 years).

You will receive several appointment reminders.

• If you need to cancel and/or reschedule, please provide 3 full business days advance notice per policy to avoid incurring an automatic $100 cancellation fee.
