Please note: Follow My Health allows for only one email address for each medical practice. In other words, if you previously used an email address at another medical practice that uses Follow My Health, you will not be able to log in. The work-around is to:

1. Ask the other medical practice to remove your email address in their Practice Fusion system and revoke access;

2. or, Use a different email address for your profile with Changing Tides.
