• Consultations: Standard consult time is 1 hour.

• Consents: Prior to your first appointment, please review and submit the signed consents using the link below, which will redirect you to a secure Updox intake form.

• Health History Form: Using the link below, please fill out the form before your consultation.

• Recent Labs: If you are recent lab results (<1 year), you are welcome to bring them with you for the first appointment.

1. The health history form can take up to 15 minutes to complete so please make sure it is done ahead of the appointment. It is thorough to ensure a good understanding of your history and concerns.

2. To keep your appointment, we must receive both forms 2 business before the date of your appointment.

Your first appointment with Kelsey is a general assessment. It lasts approximately one hour. You may bring recent lab results (<1 year) to help establish a baseline. Following the first meeting, some labs or imaging may be ordered.
